Multiple toys were precariously sitting on the slanted shelf and hanging over the edge of the shelf; 2) A small crockpot that was used as a bottle warmer was sitting on a shelf that was adjacent to, and above the height of, the sole highchair in the infant room. Interview with the staff member in the room revealed infants are placed in the high chair in the observed location, near the bottler warmer; and 3) There were ten (10) rectangular cots standing on the short side, partially blocking the bathroom door in the three-year-old classroom. Upon arrival, two (2) children were observed in one classroom with two (2) staff members. The staff members identified the children as a two-year-old child and a three-year-old. Further interview revealed each child was under the direct supervision and responsibility of one of the staff members, both from separate rooms. Observation confirmed the two children were combined in one room to begin the day, then were later moved to their respective classrooms with other children their same ages. Therefore, the center failed to keep children in the same groups once groups are established without allowing children from one group to interact with children from a different group. Also, a child in the school-age class was observed to walk through the Three's classroom to access the bathroom. There was no defined walkway through the Three's classroom that prevent children from one group to interact with children from a different group. Additionally, no verification that three (3) staff members completed the required reopening training specific to the mandated closure for the pandemic was presented for review. Observation at 8:44am revealed a white car pulling in front of the facility and five (5) children entered the vehicle on the driver’s back side and two children entered the vehicle on the front passenger's side and the car drove away. The children's enrollment dates who entered the white car are 2/24/14, three (3) enrolled on 7/1/13, and three (3) enrolled on 11/12/13. Based on interviews with the two (2) staff members who were driving the white car and the blue-green car, both reported they transported the aforementioned children to school on 1/27/15.
