General: Based on observation and interview, one- year-old children were hiding in a corner between cots and a changing table while a staff member sat in a chair singing songs to other children. The changing table was pushed against a sink leaving a gap under the sink and a blind spot due to the upright cot orientation. When the DRCCC surveyor entered the classroom it was noticed that there was one (1) child playing near the aforementioned area while also trying to stick his/her finger in an electrical outlet. The surveyor mentioned this to the staff member. When the staff member went to check on the aforementioned child, two (2) other children who had not been seen by the surveyor came from behind the changing table. The staff member made the three (3) children leave from their hiding spot. After the three (3) children ran away laughing the staff member slid the cots over to close the gap and block the electrical outlet. Upon entering the classroom, a child was standing at the door in the dark screaming and crying. The staff member working within the classroom stated to the DRCC representative that the child was “Okay, she was new.” The staff member went on cleaning the classroom. There was not one (1) time that the staff member talked to the child or tried to comfort the child. The DRCC representative left the classroom and went into another classroom. The DRCC representative was in the hallway and could still hear the child in the two-year-old room crying. The DRCC representative entered the two-year-old classroom again. The child was observed crying and had slobber and mucus on his/her face from crying so hard. Observation of the video footage revealed the staff member in charge left a ten-month-old child alone in the Infant Room, on the floor, for sixteen (16) minutes. The staff member in charge and another staff member were observed entering the room after the sixteen (16) minutes had elapsed to look around then both of them exited the room again leaving the child alone for an additional two (2) minutes. A staff member was observed bringing a second baby into the Infant Room, placing them in a swing and placing the 10-month-old child in a highchair. The staff member then walked out of the room and left the two (2) infants alone for four (4) minutes. Observation revealed three (3) additional children arriving at the center and taken to the Infant Room with one staff member, causing the number of children in care to increase to five (5) infants. The staff member working in the room was observed leaving the five (5) children in care in the Infant Room alone for four (4) minutes. Observation of the video footage revealed a staff member working alone in the Three’s Room left four (4) preschool and school age children alone two (2) times while they walked down the hallway with a one-year-old child on their hip to retrieve something from another room. The teacher was not within the range of voice or scope of vision for the preschoolers and school age children present when they left them alone. Continued observation of the video footage revealed two (2) preschool age children running down the hallway alone while the staff member in charge, who allowed them access to the building, was talking with an adult at the door. Interviews revealed that the staff members could not explain why the aforementioned ten-month-old child was left alone for such a significant amount of time.
