Such equipment consisted of a dome climber, two (2) swing sets, one (1)fire truck climbing aparatus, and one (1) set of steering wheels. Additionally, the wooden equipment, to include the bench near the steering wheels, the bench on the fire truck, and the seesaw, all contained chipping paint and non-protected wood that was chipped and splintering. The T-shaped, metal handle on the seesaw was loose. The handle spun around when touched, causing the T-top of the handle to point to where a child's abdomen would be when using the equipment. General: Based on Observation, it was determined the staff member failed to provide adequate supervision in the preschool classroom. At approximately 10:45 a.m., four children were observed playing in a separate programming space unsupervised; while six additional children were observed supervised by the staff member in a second adjoined programming space during a scheduled art activity. For approximately fifteen minutes the children were observed without adequate supervision. For example, the staff member was observed helping a child at the paint easel while her back was turned against the 4 unsupervised children in the separate adjoined programming space. Also, the staff member was observed getting paint smocks from a corner of the room which was completely out of the scope of vision. Finally at least twice, a child was observed standing at the doorway within the connecting vestibule of the adjoined separate programming spaces, completely out of the scope of vision.
