General: Based on interview and review of documentation, on 05/15/25, a three-year-old child received a cut to her chin after falling on the floor. According to video footage and interview, the victim and another friend were playing on the glass door that leads outside. The victim had her hands up hanging off of the push bar handle of the door, while another friend pulled her legs up in the air to imitate "flying". The 2 children did this twice, switching off who held whose legs, and the last time they attempted this, the victim lost her grip and let go of the bar, causing her to fall directly onto her chin on the floor. The victim had a previous injury to her chin where she had to get stitches, so this fall opened that same wound back up. Administration was immediately notified, parents were called and the victim was taken to the hospital where she received more stitches. There were 2 staff in the classroom when the incident occurred. General: Based on review of documentation and observation on 07/03/2024 the health, safety, and comfort of the children in care was jeopardized based on the following: 1. An excessive number of puzzles and puzzle pieces were strewn across the floor within the Younger Two’s classroom, creating a tripping hazard. The puzzles covered the floor to such an extent that children had to walk on top of them to move throughout the front portion of the classroom. The staff in the room did not react to this situation or assist the children. 2. A child within the Younger Two’s classroom placed their body, lengthwise, into the upper shelf of a toy storage unit, causing the unit to shake and wobble. This was observed from the hallway via the classroom’s interior window. The staff in the room were not observed providing any assistance or reacting to this incident. 3. Two (2) electrical outlets within the Older Two’s classroom were not covered or protected. 4. A can of Airwick Pure air freshener, whose label contained the phrase, “Keep out of reach of children,” was stored on a low shelf within the Older Two’s room, where it was accessible to the children in care. 5. Review of a live-stream of the center’s surveillance system over the course of approximately thirty (30) minutes revealed that a staff member in the Waddle Toddle classroom sat atop a child-sized table for the duration of the observation and did not rise to attempt to redirect or interact with the toddlers in the room. During this time, seven (7) one (1) year-old toddlers were observed walking throughout the classroom, unengaged, grabbing and touching each other, and laying on the floor while crying. An additional staff person in the classroom held one (1) child throughout the observation, but was seen looking at her smartphone frequently, and for multiple minutes at a time. 6. Continued review of the live-stream of the Waddle Toddle room revealed that one (1) child stood near the receptacle used to store soiled diapers and touched the lid and bag within the receptacle for several seconds before a staff member moved to the child to redirect them. The staff failed to ensure that the child washed their hands after touching the receptacle. Approximately three (3) minutes later, the same child was again observed touching the aforementioned trash receptacle. The staff did not react to this or attempt to redirect the child. Although space heaters were used, it was stated that the thermostat in the Infant/Waddler classroom did not read above sixty-three (63) degrees Fahrenheit at any point during that morning. Additionally, during a walkthrough of the center, conducted on the afternoon of 01/16/24, the temperature in the Four year-old classroom felt excessively cool and when a thermometer was observed in the classroom for two (2) to three (3) minutes, it did not read more than sixty-two (62) degrees during the observation. One (1) staff member cared for eight (8) school-age children in the aforementioned classroom at the time of the observation.
