General: Based on observation, on 02/23/26 a staff person was seated in a glider holding a five-month-old child. When the staff person stood up the child slipped from the staff person's arms and landed on a padded floor mat. The child initially appeared to be fine with no visible signs of injury but a small bump did show up later on the child's head. Parents sought medical attention and there were no concerns found. General: Based on review of video footage from 04/19/24 during the opening shift, two (2) staff members picked up a two-year-old child from the floor by grabbing her arms just above her elbow then sliding down to her forearm and proceeded to drag her down the hallway. While roughly bringing the child down the hall, the child was kicking and crying, and her feet and knees were dragging on the floor. Video footage from the Preschool 1 classroom showed one (1) staff member walking in first and turning the lights on, then the other two (2) entering, dragging the two-year-old child and dropping her down on the floor. The child attempted to crawl to the door multiple times, and staff members picked her up each time and roughly placed her away from the door or on the back rug. A staff member was observed snatching and pulling the child's arm when trying to keep her from crawling towards the door. A staff member was also observed pushing the child down by her back and then pushing her head back roughly after she tried biting a staff member on the leg. General: Based on observation and interview, the child care center failed to provide adequate supervision to children in care in accordance with regulatory requirements. Observation of the Intermediate classroom revealed two (2) staff and thirteen (13) children upon initial inspection of the room. Interview with staff in the classroom revealed there were fourteen (14) children and the children ranged in age from three (3) to four (4) years old. When the staff recounted the children, who rested on cots, it was then discovered that a child was missing. The staff were not aware of a child in the bathroom. The child was out of the scope of vision of the teachers in the classroom for an undetermined amount of time. Observation of the Pre-K classroom revealed sixteen (16) children and two (2) staff. Interview with staff revealed the children ranged in age from four (4) to five (5) years old. Observation of the classroom revealed a teacher in the middle of the classroom and one (1) at the entrance. The teacher in the middle of the classroom moved to the back area of the room. The surveyor stood in all positions of staff and determined that children rested in areas behind shelves, which depending on the placement of staff, could not be seen resting. The children were out of the scope of vision of staff in the center of the room and the back corner of the room.
