Interview with staff revealed that at least three (3) of the holes were caused due to a child sitting on the floor and leaning against the wall. Continued observation of the holes revealed that one (1) of the holes was patched with a compound and the compound was no longer holding the paneling and behind the paneling was a black substance as well as two (2)slugs, all of which were accessible to the children in care. Interview with several staff revealed that a two-year-old child with the enrollment date of 03/10/2014 wrapped the elastic of a cot sheet around his neck causing strangulation.
