General: Based on interview and observation, the family child care home failed to maintain equipment in accordance with regulations. The provider revealed the backyard is utilized as the outdoor play area. Observation of the back yard revealed there was an uncovered above-ground swimming pool. The provider revealed the stationary pool wall was approximately forty-eight (48) inches tall. Observation revealed there was a large, sturdy flowerpot positioned on the ground, immediately adjacent to the swimming pool. There was also a lamppost placed in the middle of the flowerpot. The height of the flowerpot appeared to be one-third (1/3) to nearly one-half (1/2) the height of the pool wall. Consequently, the height, sturdiness, and proximity of the flower pot in relation to the swimming pool created a condition whereby children could potentially access the swimming pool by climbing on the flowerpot. The provider acknowledged the flowerpot was placed next to the swimming pool. Review of the First Aid/CPR card presented for review revealed the agency which administered the training, National CPR Foundation, is not a cabinet approved agency. General: Based on Interview and Observation, the family child care home failed to maintain equipment in accordance with regulations. Interview with provider revealed the children in care use the back yard for outdoor play. Continued interview with provider revealed the children access the back yard by exiting out the front door. Observation of the back yard revealed a covered above-ground swimming pool which was immediately adjacent to the deck. Continued observation revealed a set of stairs which were not blocked which led up to the deck. Further observation revealed surveyor could walk up the stairs, on to the deck, and directly into the pool; therefore, the swimming pool was accessible to children in care.

General: Based on Interview and Observation, the family child care home failed to maintain equipment in accordance with regulations. Interview with provider revealed the mats used by children in care for rest time were stored in the closet. Observation revealed two (2) of the mats were torn at the seams.

General: Based on Observation and Review of Documentation, the family child care home failed to maintain food service in accordance with regulations. Observation of the home revealed an eighteen-month-old in the care of the provider at the time of survey. Review of the menu presented for review revealed skim milk was listed on the menu for breakfast, lunch, and snack for the entire week; however, pasteurized whole milk was not indicated on the menu as required for a child age twelve (12) months to twenty-four (24) months.
