General: Based on Interview, the childcare center Director failed to maintain the health, safety and comfort of all children in care. In an interview with the Director, a staff member and a parent, it was determined that an infant did not have a bruise on his ear upon entry of the childcare center on 05/11/2015. Continued interview with the Director and a staff member revealed that the infant sustained an unknown injury to the ear causing a bruise from the time that the infant was dropped off by his parent to the time the infant was picked up. Review of documentation revealed that a staff member with the hire date of 02/24/2014 did not have an out of state background check presented for review. Continued review of documentation revealed that a high school diploma from 2011 and a Child Abuse and Neglect Check both had an Ohio address. Review of attendance records from 05/11/2015 revealed that nine (9) children were present in the center, however, one (1) child was neither signed in nor out and another child was not signed out of the center. Observation of the center on 05/18/2015 revealed that seven (7) children were in attendance. Review of attendance sheets revealed that only six (6) children were signed into the center.
