Review of attendance records from 05/11/2015 revealed that nine (9) children were present in the center, however, one (1) child was neither signed in nor out and another child was not signed out of the center. Observation of the center on 05/18/2015 revealed that seven (7) children were in attendance. Review of attendance sheets revealed that only six (6) children were signed into the center.
