General: Based on Review of Documentation and interview, the child care center failed to maintain the food service in accordance with regulatory requirements. Review of the posted menu failed to reveal a menu for the current date. The June menu that was posted failed to be accurate with the calendar and failed to contain a menu for the date of the survey (06/29/17). General: Based on Review of Documentation, the child care center failed to maintain children's records in accordance with regulatory requirements. A review of documentation revealed a file for a child enrolled on 05/04/17 failed to have enrollment information including a preferred hospital, a contact number for a person in charge to contact the child's physician, persons designated in writing to pick up the child, and an authorization by the parent for the child-care center to seek emergency medical care for the child in the parent's absence.
