General: Based on observation, the surveyor found the following: 1. Upon arrival at the child-care center, the surveyor conducted an initial headcount of each classroom, starting at 1:50 pm. The surveyor observed that the Preschool Classroom #1, Preschool Classroom #2 and the Two's Classroom did not have adequate light in the classroom and the surveyor was unable to determine how many children were in the classroom. The surveyor asked each staff for a headcount. At 2:13 pm, the surveyor stood in the doorway of the Two's Classroom and observed the staff person sitting in a chair, with her back to the children and on her phone. The surveyor stood in the doorway for two (2) minutes, re-counted the children and found that there were seven (7) children. The surveyor entered the classroom, and the staff person then placed her phone on the floor. 2. The surveyor observed a four-year-old child exit the Preschool Classroom #1 at 2:00 pm and 2:04 pm, walk down the hallway and into the front office unattended. The surveyor observed during this time the staff person responsible for Preschool Classroom #1 to exit the classroom, shut the door behind her and walk behind the child to ask her to come back to the classroom. The surveyor found during this time that the staff person was leaving Preschool Classroom #1 unattended both times that she exited the classroom; therefore, the child-care center did not provide adequate supervision. 3. While the surveyor was sitting in the front office reviewing paperwork, the surveyor overheard a child from the Preschool Classroom #2 if he could go to the restroom that was located across the hallway. The surveyor observed the child to walk from the classroom to the restroom, shut the restroom door and stay in the restroom from 2:51 pm - 2:53 pm. The surveyor observed a staff person to be behind a half door, that was located across the hallway in the Preschool Classroom #2 area however, the child was observed to be out of range of voice and out of range of vision of the staff person. The surveyor did observe the staff-in-charge walk over to the door area, where she asked the surveyor if she was waiting for her. The surveyor replied that she was in the hallway due to a child being in the restroom and the surveyor was taking the time and notes. The surveyor confirmed with staff-in- charge that the child in the restroom was a four-year-old who attends only on Fridays.

General: Based on observation and interview, the surveyor found the following: Playground: The surveyor found that there was a large sinkhole area in one section of the Preschool/School-age playground. The sinkhole was observed to be large enough to show the underground plumbing and was observed to give way to outside of the fenced area. In an interview it was found that the hole had been on the property since May 2025. Front Office: Upon arrival at the child-care center, there was a child sitting in the front office looking at the computer screen. When the surveyor asked why the child was sitting in the office, the surveyor was informed it was because she did not like to nap. The staff-in-charge reported that she had been in the front office for approximately ten (10) minutes and was not alone. The surveyor observed the child to enter Preschool Classroom #1 at 1:56 pm. The surveyor reviewed the child's file and found that she was four-years-old. Restroom #1: There was a bottle of "Poo Polish" and a bottle of "Lamimpress Coconut Cream Perfume" placed on top of the paper towel holder. Both items had a label on the back that stated to keep out of the reach of children. Preschool Classroom #2: There was a long red extension cord that was placed on a shelf. Therefore, the health, safety and comfort of the children was compromised.

General: Based on review of documentation, the surveyor found the following: 1. A staff person's (DOH:06/26/24) file contained a negative tuberculin result that was no longer current after 05/24/25; therefore, the child-care center failed to show proof that the employee was free of active tuberculosis or provide a copy of a negative tuberculin result. 2. When the surveyor returned to the Infant Classroom to conduct a room check at 2:22 pm, the six-week-old was still asleep in a crib with a scrunched-up sheet and lying on his belly. The surveyor asked the staff in the classroom how long the infant had been asleep and the surveyor was informed that he had been asleep since 12:30 pm. The surveyor observed there to be a poster above the crib with tips on how to place an infant in a crib for safe sleep.

In the hallway with the diaper changing station, the following was found: 1. There was a container of Clorox wipes placed on top of a small shelf that was within reach of children. 2.
