General: Based on interview and review of documentation, on 07/19/24 a two (2)-year-old child was left unsupervised on the toddler playground from approximately 3:23 pm until 4:07 pm. This was observed on video footage provided by the facility. On 07/19/24 three (3) children were taken to the toddler playground by a staff member to combine classes (approximately 2:54 pm). There were two (2) classes on the playground. Based on interview, the staff member that moved the children indicated that she verbally told the teachers on the playground that these children would now be part of their class. The teachers took the children inside at approximately 3:23 pm. One (1) child was left outside unattended on the playground after the other children went inside with the teachers. The video showed the two (2)-year-old child playing on outdoor equipment, followed by the same child trying to enter a door to the building. The facility's doors automatically lock when closed, preventing outside entrance. The child was found by another staff member that was leaving work in her car, drove by the outdoor play area, and saw a child on the playground alone. The staff member called a staff member inside the building at approximately 4:07 pm and the child was immediately taken inside. The child was never missed by either staff member that were supposed to be supervising the child. The child did not sustain any injuries. Based on interview, the child was picked up by the parents at approximately 5:07 pm on 07/19/24. In Classroom #5, the surveyor observed two (2) stacks of six (6) child-sized chairs positioned next to the children’s cubbies. The children present in the classroom were age two- years-old. On 7/22/2022 at approximately 4:00 p.m., a three-year-old child was left unattended in classroom #7 for one minute and twenty-eight seconds. A review of video footage found the child was in the restroom as staff and children exited the classroom. Interviews found that one staff person and nine children transitioned from classroom #7 to classroom #8. The second staff person transitioned to classroom #9. Both staff stated that they did not do a 'name to face' count of the children or check the restroom prior to exiting the classroom. General: Based on review of documentation, on 03/28/2022, video footage showed a staff person treated a two-year-old child inappropriately in a classroom. The staff person was sitting on the floor with several children gathered around her playing with blocks. At one point a two-year-old child is seen sitting on her knees directly behind the staff person. The staff person reaches around behind her, while still sitting on the floor and grab the child right above the left elbow and drag the child approximately 2 feet so the child is next to the staff person. The child who was still on her knees loses her balance and catches herself by placing her right hand on the floor. The child then stands up and is face to face with the staff person. The staff person is then seen shaking her finger very close to the child's face and appears to say something to the child. The child then walks away but it appears the staff person is still upset because she is then seen forcefully grabbing an object from a child and placing it between her legs. The staff person is also seen grabbing blocks away from children in an aggressive manner. A second staff person stated that she had seen and heard the staff person in question get loud and be somewhat impatient with the children in the classroom. The staff person in question stated that she did feel as if she was a little too aggressive with the children at times but is now more patient with the children. The staff person in charge stated that the staff in question was placed on probation and was given a coaching session. It should be noted that a prior incident happened on 02/14/2022 with the staff person in question regarding tone of voice and aggression towards children in the classroom.

General: Based on interview and review of documentation, on 09/09/21 a two-year-old child was left alone on the playground without adult supervision from approximately 10:45am- 11:00am. At approximately 10:45am, per video survellience and interview with staff, there were two toddler classes on the playground. Room 6A had ten (10) children with one staff and Room 6B had nine children with one staff and a speech therapist. As Room 6A's class was coming inside from the playground, an eleventh child was dropped off and was given to Room 6A's staff person which then gave her a total of eleven (11) children. Room 6A staff person told Room 6B's staff person that she was leaving one of her children on the playground with Room 6B so they both would then have ten (10) children. Room 6A then went inside the building. A few minutes later Room 6B came inside. In the video footage there are nine children that came inside with Room 6B and the speech therapist instead of ten (10) children. Per interview, Room 6B staff person asked Room 6A staff person at approximately 11:00am if she had seen the child in question and Room 6A staff person reminded her that she now had ten (10) children instead of nine children and that she did not have the child in question in her classroom. Per vidoe surveillance, after realizing the child was not in the classroom, Room 6B staff person opened the door leading out onto the playground and let the child back into the building.
