General: Based on Review of Documentation, the submitted plan of correction regarding the use of portable sinks in two (2) of the classrooms indicated that as of July 1, 2015 the facility budget allows for an additional staff member to work in the rooms with portable sinks if hand washing needs an alternate location ratios can be maintained at all times. The plan of correction also indicates reinvesting in heating elements and are considering installing permanent sinks. However, at the time of the visit conducted on 7/7/15 upon entering the Crawlers room a staff person was observed washing a child's hands at the portable sink. There were three (3) staff in the room. The surveyor followed the child into Room #207 and observed two (2) staff caring for fourteen (14) children. One (1) staff person was observed standing at the door inside the classroom. The classroom door was closed while the child was observed in the hallway and in the bathroom; therefore, adequate supervision was not provided. 2. A second incident of inadequate supervision was observed with the same group (Room #207) during the same time frame (approximatley 9:20AM). The staff person that was standing at the door when the surveyor entered the classroom exited the classroom with a small group of children to take them to the bathroom and then re-entered with the children a few minutes later. When the surveyor exited Room #207 to continue a tour of the building a male preschool child was observed running out of Room #206 at the opposite end of the hallway and heading back into Room #207. The surveyor observed the staff person that had escorted children to the bathroom stop the child in the hallway and ask him why he was coming out of Room #206.
