The other group had seventeen (17) children ages two (2) to three (3) years old with one (1) staff person responsible of the group. Groups were mixed together while playing on the playground. The maximum group size with the age of the youngset child being two (2) years old is twenty (20) and the staff to child ratio with the age of the youngest child being two (2) years old is ten (10). The groups size was over by seven (7) children and the staff to child ratio was over by seven (7) children. There was another staff person inside the building who was supposed to be with the group of seventeen (17) children but she was inside at the time and came outside with the surveyor.

During a tour of the facility at approximatley 9:10AM, the surveyor walked upstairs to the preschool hallway and observed a male child standing alone in the hallway in front of the bathrooms. When the child saw the surveyor he turned to get a drink at the water fountain and then went into the boys' bathroom. The surveyor observed the child exit the bathroom approximately one (1) minute later and then enter Room #207 (four (4) - five (5) year old classroom). The surveyor followed the child into Room #207 and observed two (2) staff caring for fourteen (14) children. One (1) staff person was observed standing at the door inside the classroom. The classroom door was closed while the child was observed in the hallway and in the bathroom; therefore, adequate supervision was not provided. 2. A second incident of inadequate supervision was observed with the same group (Room #207) during the same time frame (approximatley 9:20AM). The staff person that was standing at the door when the surveyor entered the classroom exited the classroom with a small group of children to take them to the bathroom and then re-entered with the children a few minutes later. When the surveyor exited Room #207 to continue a tour of the building a male preschool child was observed running out of Room #206 at the opposite end of the hallway and heading back into Room #207. The surveyor observed the staff person that had escorted children to the bathroom stop the child in the hallway and ask him why he was coming out of Room #206. The staff person was not aware that the child had gone into Room #206; therefore, adequate supervision was not provided.
