Upon arrival at the center at 1:35 pm, the surveyor observed the center to be out of staff-to-child ratio and exceeding the group maximum. During nap time, the toddler and preschool classrooms were combined in the hallway/school-age classroom. There were fourteen (14) children present with two (2) staff. The youngest child was one (1) year old and the oldest child was five (5) years old. Based on the ages of the children present, the required staff-to-child ratio and group maximum were exceeded by two (2) children. The person in charge reported that the classrooms are routinely combined from 1:00 p.m. until 3:00 pm during nap time. General: Based on observation and review of documentation, at the time of the follow-up inspection, at approximately 9:40 am, the surveyor and staff member in charge were in the process of touring the facility. During this time the staff member in the Preschool Room left seven (7) children ages three-to-five years old unsupervised while she went into the office to answer the phone. The staff member brought the phone back into the classroom with her. At that point the staff member in charge verbally reprimanded the staff member for leaving her classroom unsupervised to answer the phone. The staff member left the children unsupervised less than a minute. It should be noted that during the renewal survey conducted on 12/09/24 supervision was cited when a staff member left four (4) children ages two-to-four years old unsupervised sitting on the carpet outside of the bathroom while the staff member left the area to retrieve paper towels. The plan of correction indicated staff members have been directed to gather children and take them with staff when there is a need to leave classroom or anywhere else. General: Based on observation and interview, at the time of the follow-up inspection a tour of the facility found the following: 1) A stairwell within the Preschool Room had stairs that were not protected, allowing the children access. 2) A fluorescent light fixture located in the hallway was not covered. An interview with the staff member in charge found a child safety gate had been placed at the bottom of the stairs in the Preschool Room. However, the gate was unstable and would not stay in place and had been removed two (2) days prior to this inspection. The staff member in charge stated further that a new gate which will be attached to the wall is being installed on Friday of the current week. An interview with the staff member in charge also found a covering had been purchased to replace the cracked cover on the uncovered light fixture. However, the cover did not fit, and a new one is being purchased and will be installed. It should be noted that these violations were found during the renewal inspection conducted on 12/09/24. Upon entering the School-age space, the surveyor observed three school-age students sitting at a desk. There was also a child in a high chair and a toddler walking around within the same area. The toddler ran out of the school-age space and down the hallway, out of sight. The staff member left the school-age children and the child in the high chair to go retrieve the toddler. The toddler also ran into the Two-Year-old classroom and began to run up the steps. The staff member was able to retrieve this child once more. A few moments later, the staff member was observed to bring an infant from the Infant classroom; it is unknown how long this child had been left unsupervised in the infant area. The staff member left the children on at least two other occasions to retrieve milk from the refrigerator, and to retrieve the running toddler. Shortly after, another toddler arrived and joined the group. The staff member was then observed to take four infant and toddler age children into the Two-Year-Old classroom, leaving the school-aged children in the school-aged area unsupervised. The staff member stated that she was responsible for the Infant classroom, the Toddler classroom and the School-aged classroom since the regular staff was not present. The staff person stated during interview, "It was between 1:00PM - 1:30PM and I was trying to get the child to sit down and be quiet, but he was fighting me. I hit the side of my hip on the large support column in the classroom while trying to get him into a chair and I grabbed him because I was falling. I thought I was going to fall in the floor. He was twisting and turning in such a way that I had to wrestle with him to get him to sit down. I grabbed his left arm and then tried to grab him right arm. This is when I almost fell. I finally got him in his seat and pushed his chair up to the table". The staff person described the child's behavior during this incident by stating, "He was crying, growling and throwing his hands up in the air so I couldn't grab them". 2. On 3/2/17 at 7:06AM the surveyor observed a staff person and three (3) school age children walk out the front door of the church and a garbage truck pull up in front of the church at the same time. At this time, the three (3) school age children walked across the street by themselves and down the sidewalk (approximately one (1) hundred feet) to the bus stop while the staff person ran through the parking lot to the back side of the church to get the garbage containers. The staff person had to run to the back of the church two (2) times to roll out two (2) different trash containers to the garbage truck. After the garbage truck emptied the containers, the staff person rolled the two (2) containers to the back of the church and went inside. The staff person never returned to the front of the church. This whole process happened between 7:06AM - 7:11AM. The three (3) school age children were still observed standing on the sidewalk when the staff person went inside. The children were observed getting on the school bus at 7:12AM. It is important to note that the three (3) school age children were observed standing on the sidewalk with an unidentified man and two (2) other children. The surveyor entered the facility at 7:13AM and observed the staff person identified as the one moving trash containers, assisting two (2) boys (three (3) year olds) in the children's bathroom area near the front door. The surveyor asked the staff person if any other children or staff were present in the facility. The staff person responded "No". The surveyor asked the staff person where the two (2) boys were while she was out front. The staff person stated, "I left them standing at the front door of the church because they knew I was coming right back". The director revealed a situation that occurred the previous week by stating that the adult male was on the van while she picked up children from school one day and stated that when the van arrived at the facility, the adult male opened the side door for the children to exit. General: Based on Review of Documentation, it was found that three (3) children's files contained immunization certificates that were no longer current. These immunization certificates expired on 5/15/11, 6/30/11, and 8/31/11. All three (3) of the children had been enrolled in excess of thirty (30) days.
