The fan cover contained holes, and a child could access the blades of the fan. Upstairs Gross Motor Room 1. The climbing structure with a slide was located against the wall. General: Based on interview, it was revealed that a staff person did not utilize developmentally appropriate programming techniques when working with toddlers. Three staff reported seeing a fellow staff member spray toddlers with a water bottle when they would not comply with the staff member’s requests. Two of the staff reported their observations to management on 3/4/2021. The first staff person stated that she observed the staff member spray four toddlers in their faces when they would not do what was asked. A second staff person stated that she too had observed this behavior for a couple of weeks, and that several toddlers had been sprayed in the face. A third staff person observed the staff member spray the toddlers on two occasions. The first time, it appeared to be just a game; however, the second time it was directly in a toddler’s face for climbing on a table. An interview was conducted with the staff member on 3/31/2021. The staff member admitted to spraying a toddler with water in order to get her off a table, and spraying mist over toddlers’ heads as a game.
