General: Based on observation, the childcare center failed to maintain an outdoor play area in accordance with regulations. Observation of the outdoor play area revealed three (3) large gaps/holes between the wooden perimeter privacy fence and the ground, thus creating multiple entrapment hazards. Further observation of the outdoor play area revealed a large section of black weed barrier protruding up from the base of a tree. Continued observation of the outdoor play revealed a large tree branch lying on an adjacent sidewalk that leads to the access gate of the outdoor air conditioning and heating units. The branch was within reach of students as they enter the play area.
