General: Based on observation and interview, at 10:04 AM the surveyor entered the Two-Year-Old Classroom and observed one (1) staff member changing a child’s diaper. The staff member stated that she left her co-staff on the playground with eleven (11) children aged two (2). The surveyor observed one (1) staff member caring for eleven (11) children on the playground. The second staff member rejoined the group at 10:09 AM. At the time of survey, ten (10) infant aged children were observed being cared for by one (1) staff member for approximately one (1) minute. The staff to child ratio for an infant classroom is one (1) staff member for five (5) children. In the toddler classroom, two (2) staff members were observed caring for thirteen (13) children which exceeds the minimum ratio. The children were one (1) year old. The maximum group size for one (1) year old children is twelve (12). In the outdoor play area, thirty-eight (38) children were found being cared for by three (3) staff members. Based on interview, one (1) staff member had placed a child in care in time out while standing and facing a wall. In the section labeled Discipline of Children, the written policy of the facility states, “at no time can a teacher put a child standing in time out with the child’s face/nose facing towards a wall.” During staff interview, a staff member reported that a child had been placed in time out while standing and facing a wall. Based on interview, one (1) staff member reported that socks had been placed on a child’s hands to prevent the child from hitting other children. The staff member reported that the socks had been placed on the child’s hands on more than one occasion. The staff member also reported that the socks are usually kept on the child’s hands for no more than fifteen (15) minutes at a time. The staff member stated that the socks were never placed on the hands of other children. The facility written policy states, “No child shall be teased, humiliated, frightened, ridiculed, or belittled at any time. No child shall be shamed.”

The staff member reported that a child in care was made to stand in time out while looking at the paper taped to the wall. The staff member reported that the child was made to stand for no more than five (5) minutes. The staff member reported that only one (1) particular child is made to stand for time out.
