General: Based on observation and interview, this regulatory requirement was not met. Upon arrival, the surveyor toured the facility with the staff person in charge. Eighteen children (three-to-five-year-old's) and one staff person were observed present in the Pre-K Classroom at 8:03AM. The staff person in charge stated that the preschool teacher, who arrived to work at 8:00AM, would be pulling the four preschool children from the group and taking them to her classroom. The staff person in charge remained in the Pre-K Classroom for ratio purposes until the preschool teacher arrived in the classroom. Since the youngest child in the group was a three-year-old, the minimum staff to child ratio would need to be one staff person for twelve children. Upon review of attendance records, it was found that there were seven days when the infant one classroom was over group size and ratio. On these days, the number of infants varied from 11 to 14 at any given time; therefore, not meeting the maximum of 10 infants per group. When reviewing the attendance records for the infant two classroom, it was found that there was no documentation indicating that the class was over group size; however, it was found that at certain times throughout the day, there would be up to nine children in the classroom. The attendance records did not indicate what teachers were present and at what time they arrived. Interviews with the infant two teachers indicated that the room was over ratio at times and had too many children with only one teacher present.
