On 9/1/21, the facility reported that a one-year-old child was accidentally left alone in the Toddler B Classroom for approximately 45 minutes when her group went outside to play. Staff stated during interview that the two staff assigned to the group had eight children present in their group at 10:00AM and they took the group outside. One staff person was assigned to cover a staff person's break in another classroom at 10:30AM, so she left the group at this time and took two children from the group with her. Therefore, one staff person was left to care for the remaining six children. At 10:45AM, this staff person returned back inside to the Toddler B Classroom from the playground. At this time, an administrative staff person was walking through the Toddler B Classroom to get to the Infant B Classroom and noticed a female child asleep on pillows in the corner of the classroom. The administrative staff person immediately brought the situation to the staff person's attention and it was discovered that a name to face count was not conducted by staff when they exited the classroom to go to the playground. The staff person was terminated as a result of the incident. It is important to note that the Toddler B Classroom and the Infant B Classroom are separated by a half wall. There were two staff present with a group of children on the Infant B Classroom side; however, they were not aware of the child sleeping on the other side of the wall and she was not visible to them. Interviews with staff found that at approximately 3:15 p.m. the staff person in Toddler A had six (6) children in her care. She used the classroom I-Pad to do a named to face count of children before she took her group outside and then handed the I-Pad over the half wall in the room to a staff person in Toddler B. She then took the children outside. The staff person indicated in interview that each classroom has their own I-Pad to document a name to face count of the children in their room every half hour and anytime they transition from one (1) hour to another such as going outside and inside. However, some of the classrooms I-Pads had lost power and they had to share with another classroom. Thus Toddler A was sharing with Toddler B. The staff person stated she took her children outside noting that three (3) of her children are not walking so she must carry them in and out. Sometime between 3:50-4:00 p.m. the staff person in Toddler A was bringing her children back inside. One (1) child remianed outside on the playground with her father who had come to the facility to visit. The staff person got the non-walkers to the door, which based on observation is hidden by a tall wood fence that encloses the air conditioning unit, and then she called for the walkers to follow. The children were following the staff person inside. However, when the staff person got the children inside and at the table to drink their water the staff person noticed there were only four (4) children noting that one (1) child was outside with their parent. The staff person asked for the I-Pad from staff in Toddler B who were going outside. The staff person stated she noticed through the window located in Toddler B that the child she was missing was still outside and was sliding down the slide. She also saw that the parent who had come to visit his child was still outside with his child as well. A staff person in Toddler B went outside and got the child who had not come in with the group and gave him to the staff person in Toddler A. Interviews found that the staff person who left the child outside was sent home and received a written warning. The child was left outside somewhere between five (5) and ten (10) minutes or possibly less. The amount of time varied per staff interviews.
