After interviews it was determined that staff members witnessed Child #1 with no injuries prior to entering the playground. During pickup on the playground, it was discovered that the child had a bruise and knot on the face near his eye. No one witnessed the injury and no one knew how the child sustained the injury.

General: Based on observation and interview, it was found that a staff person was playing "Barber Shop" with a one-year-old child on 9/1/2020. In video footage viewed by the DCBS representative, it was found that the staff person was sitting on the floor with the one-year-old child in her lap. The staff person could be seen playing with the child's hair, similar to a pulling/picking motion. The staff person's hand can be seen moving around the child's head but cannot make out if there is something in her hand. Eventually, the child becomes visibly upset and moves away from the staff person. During an interview, the staff person stated that she was using adult scissors to pretend to cut the child's hair. The staff person indicated that she accidentally cut a patch of hair out of the back of the child's head. A photo provided by DCBS of the back of the child's head shows a bare patch approximately three to four inches in diameter.
