During a tour of the facility the following was observed. 1) A toilet plunger was found by the toilet in the first stall in the boy's bathroom located near the facility office. 2) A toilet plunger was found by the toilet in the first stall in the girls' bathroom located near the facility office. 3) In the Toddler II room there were large vinyl blocks in various shapes for children to walk and climb on. These blocks were observed in the corner of the room near a low wooden shelf. One (1) of the blocks, shaped like a triangle, was placed on the floor to form a ramp to the other blocks. Some of the children were observed running down the ramp. Due to the location of the end of the ramp and the position of the shelf there was not enough space to keep a child from hitting the shelf if they should happen to fall. General: Based on Observation, during a tour of Toddler II there were six (6) children ages one (1) to two (2) years old in the room. An interview with staff found that three (3) of the children are two (2) years old. During an interview with the staff person in charge it was indicated that these children are enrolled in the classroom and remain in the room all day. The staff person in charge also indicated that there is not room to move the two (2) year old children into an older room. The staff person in charge stated further becasue they operate during the school year only she did not want to move the children to another room for only a few months. Lunch was observed being served in the Early Preschool room. The following individual lunches were observed that did not have the required components for lunch; corn, orange, bagel with cream cheese and watermelon fruit juice; pretzels, popcorn and strawberries; macaroni and cheese, yogurt and a roll. An interview with the staff person in the room found that if the child did not bring anything to drink or brought somthing other than milk they will offer them water to drink with their meal. An interview with the staff person in charge found that they had cheese sticks and bananas available for those children as a suppliment to those lunches that did not meet the requirements.
