During the investigation it was found that a staff member let a four-year-old child go to the bathroom alone, around 12:50 pm on 6/30/2022. After approximately five (5) minutes, when this child did not return to the room, staff member went to look for him and discovered that he was not in the bathroom. At that time, the staff in the facility started the search for this child. At approximately 12:59 pm another staff member, who was in the van with her class on the way to the field trip, spotted a child running across the main road and called the facility to make sure all their children are there, and learned that staff members are looking for a child. The staff members in the van came to the child on the side of the road and were able to return him to the facility approximately at 1:06 pm.
