Based on record review and interview the facility failed to report an incident which involved medical treatment to the cabinet within twenty-four (24) hours. A facility incident report was received in the DRCC office via fax on 10/10/11 which stated, "On 9/27/11 at 5:20pm a ten (10) year old child was sitting on the gym stage when she fell off and landed on her left arm." Staff interview during the investigation revealed that the child broke her arm as a result of the fall. The staff person stated that the incident was not reported to the DRCC office within the required timeframe because they had to wait several days on paperwork that was being filled out by the child's parents.
