General: Based on observation, the following hazards and maintenance concerns were identified: 1) The wooden deck leading to the outdoor play area was in disrepair. The wooden border surrounding the deck was separated from the framing, exposing rusted nails. 2) A portion of the wooden fence in the backyard was leaning inward and was being supported by wooden beams. 3) A section of the wooden trellis on the back patio was deteriorating, creating gaps between the fence and the patio. 4) There were no barriers in place to prevent children from accessing the area behind the shed in the outdoor play area. Access to this area reduced visibility for supervision. 5) An electrical outlet box located behind a bench on the wooden patio was not secured to the wall. The unsecured fixture exposed interior wiring and presented a potential shock hazard. 6) A river-rock water fountain was located in the backyard, with no barriers in place to prevent children from accessing it.
