General: Based on observation, at approximately 2:56, the surveyor observed five (5) school age children exit a school bus and enter the facility. A female staff member was observed holding the door open as the children entered the building. The surveyor then entered the building and identified herself to the staff member standing at the door and the purpose of the visit. It should be noted that as the surveyor entered the facility, a van was observed entering the parking lot that bared the name of the facility. Children were observed on the vehicle. The surveyor asked the staff member if the children who had been observed getting off the bus and entering the building were in the back of the facility. The staff member confirmed the location of the children but did not follow the surveyor and remained at the front of the building. At approximately 2:58 pm as the surveyor walked down the hall to the back classroom and a female school age child was observed exiting the bathroom located to the right of the hallway. The child proceeded down the hallway and entered the classroom located at the end, joining four (4) other school age children who were already inside the classroom. While in the classroom, the surveyor observed another female child exit the classroom and walk down the hallway to the bathroom unsupervised. The surveyor observed there was not a staff member present within the classroom with the children or in the hallway with the children. The surveyor observed further that the only staff member within the building at that time was the staff member who remained at the front of the building. It should be noted that the child in the hallway and the children in the back classroom were not within scope of vision or range of voice. At approximately 3:03 pm there still was no additional staff member inside the building. At approximately 3:00 pm, the van driver and nine (9) school age children entered the building. The children were not escorted down the hallway to the classroom as they entered the building. Some of the children were observed stopping in the hallway and entering the bathroom before going to the classroom. At that point the children still were not supervised by scope of vision or range of voice until the van driver entered the classroom at approximately 3:05 pm. However, the van driver remained within the classroom, several children were observed exiting the classroom and walking down the hallway to the bathroom, leaving the children out of scope of vision and range of voice. This violation was cited during the renewal inspection conducted on 12/11/24. The plan of correction indicated that two (2) teachers would be present at all times. The plan of correction indicated further that one (1) staff member would remain at the door to sign children in and the other staff member would escort the children back to the classroom to put their belongings in their cubbies. The staff member signing the children in would then escort the remaining children to the classroom after all children were signed in. General: Based on observation, interview and review of documentation, during a follow-up visit conducted on 03/21/24, a gray vehicle pulled up in front of the facility. Two (2) children exited the back of the vehicle and entered the facility. The adult driving the vehicle was identified as a staff member. Further interview with staff found the vehicle is the staff members personal vehicle and was used to pick the two (2) children from Tates Creek Elementary and transport them to the facility. A review of the Division of Regulated Child Care (DRCC) records only shows approval of a 2001 gray Dodge Ram fifteen (15) passenger van and a red 2000 Dodge Ram fifteen (15) passenger van for use in transporting children. The personal vehicle observed transporting children on, 03/21/24 was not noted as an approved vehicle for transportation. Only the two (2) fifteen (15) passenger vans have been approved. An interview with the staff member in charge found the red van is still in the shop for repairs which is why the staff members personal vehicle has been used to transport the two (2) children from Tates Creek Elementary. It could not be determined specifically as to how long the staff member has been using their personal vehicle to transport children. It was estimated approximately two (2) weeks.
