General: Based on observation, the following was found: 1.) A highchair was not equipped with the manufacture’s seat belt. 2.) A set of porch steps in the outdoor play area were not blocked. 3.) At 11:17AM, several children were sent to the restroom down the hallway to wash their hands. The children were not within line of sight when sent to this area. At 11:18AM, the staff person in charge left the main child caring space to stand in the hallway between the restroom and the main child caring space. The staff person was able to visually monitor the children who were washing their hands; however, two children were not able to be supervised while in the main child caring space. One child was sleeping on a chair and another child was left in a highchair. The staff person re-entered the main child caring space at 11:20AM. General: Based on interview and review of documentation, it was found the provider has two dogs and two cats. A review of documentation found no evidence to verify the two cats are currently vaccinated against rabies. General: Based on observation at the time of the initial inspection, there were three school-age children present. None of these children were observed wearing a mask or face covering. This is in violation of the Governor's mandate that all children over the age of five wear a mask.
