General: Based on observation, the outdoor play area adjacent to the Head Start A Room contained an area of playground surfacing that was breaking apart. In addition, an area of the surfacing in the same outdoor play area had buckled into a small mound that posed a tripping hazard.

When the child pulled on the fence, it fell on top of her. The fallen fence/gate caused injury to the child's head. General: Based on observation, this regulatory requirement was not met. The basement of the facility contained a large concrete pit (in the gym) that previously was a foam pit for a gymnastics room. The facility is currently using the area for storage. The pit was approximately five feet deep. The pit was concrete and contained a small amount of water at one end. The area contained a tall ladder and some yellow caution tape around one end. The facility sent a photo via email (after the renewal visit) showing that the water had been removed from the pit. This room is located in the basement, which is down the stairs, then down the hall to a set of double doors, that leads to the hallway, then through another door which leads to the storage room (gym). None of the doors were locked; therefore, the area was accessible to children. The concrete pit is a safety hazard for the children. General: Based on observation and interview, this regulatory requirement was not met. The following was found: 1. The surveyors observed a piece of carpet (approximately two feet by two feet) lying in the corner of the room in School-Age Room #1. The carpet was covering up an unfinished area of the floor and was not attached. 2. The surveyors observed that a strip of carpet and baseboard was missing along the full length of the sink cabinet (approximately nine feet long) in School-Age Room #2. The carpet was not cut to fit nor secured to the floor. The staff person in charge stated during interview that they were waiting on new sink cabinets to arrive in both of these classrooms before finishing the flooring.
