General: Based on interviews, the surveyor found that on 8/23/2022 at approximately 2:30 p.m., a four-year-old child was left in the restroom when her class transitioned to the playground. Staff in the adjacent three-year-old classroom saw the child in the restroom washing her hands. The child said hi to the staff and exited the restroom. She soon returned and told the staff that she did not know how to get outside. The staff realized that the child never went outside with her class and immediately escorted her to the playground and informed her teachers. The staff person in charge confirmed via video footage that the child never went outside with her class. The child was in the restroom/ classroom for approximately seven (7) minutes. The four-year-old staff stated that they miscounted the children and never checked the bathroom stalls prior to going outside. Staffing Requirements In Compliance

Review of video footage found that the classroom entered the side door of the facility at 11:14am and the child was observed to be brought back into the facility at 11:38 am. In an interview with the Manager, she stated that the teacher who left the child on the bus had been standing outside of the bus with a group of children for about four (4) minutes prior to them entering back into the facility. The staff in charge of supervising the child stated that she and another staff person took a group of children out to the vehicles to practice loading and buckling the children before an upcoming field trip. This staff stated they buckled the children in and then had the children unbuckle their seatbelts. The staff stated she did not remember going to the back of the vehicle to check that all children had exited. The child was not discovered missing until later when lunch was served. At that time she was found strapped in the car seat, crying.
