General: Based on observation, the following was found: Infant Classroom 1.) One (1) crib contained a loosely fitted crib sheet. This sheet was gathered and bunched up around a child's face, causing the sheet to be covering one side of the child's face. The child was sleeping on his stomach (the child could roll over on his own) and you could only see one side of the child's face due to the crib sheet. Red Room #2 1.) General: Based on observation, the following was found: Preschool Classroom 1.) There was a teachers tumbler style cup accessible to the children in care. This cup was sitting on a wheeled cart that that contained drawers with different colored bins. Cleaning Closet (Across from Orange Classroom #2) 1.) This closet was not locked during the surveyors tour of the building. The closet contained numerous cleaning materials and chemicals, such as glass cleaner, a mop bucket, and neutral floor cleaner. Hallway Across from Pre-K Classroom 1.) There was a bottle of Sink and Surface Cleaner sitting on a sink that was accessible to the children in care.

The child size table located closest to the entrance of the classroom was dirty with black stains. Orange Classroom #2 1.) There was a green vinyl shaped star (child size) used for children to sit on that contained tear around each corner; some corners more significant than others. This exposed the interior padding and foam. Orange Classroom #1 1.) There was a green vinyl climber that contained a tear at bottom, corner. Red Classroom #1 1.) There was a diaper changing table that was damaged from where something has been taken/peeled off of the changing table.

General: Based on interview and review of documentation, on 10/15/24 a two-year-old child was left in soiled clothing for approximately an hour before the child was changed. Based on interviews, the child did not have a change of clothes in her cubby, so the clean clothes were obtained by another staff member. Based on interviews, the child was taken to the bathroom (along with the rest of the children in the class), but the staff member forgot to put the clean clothing on the child, and she put the soiled clothing back on the child. The class then went outside where it was brought to the staff member's attention that the child's clothing was not changed into the new clothing that was provided. Based on interview, the child was in the soiled clothing for approximately an hour.
