General: Based on Interview, a visit was attempted on 1/11/16 but it was found that the director was not present. However, an interview with the director's husband who was home at the time found that the program operates from 3:40 p.m. - 5:00 p.m. Monday through Friday and from 7:30 a.m. to 5:00 p.m. on days school is not in session including summers. A review of information in KICCS found that the hours listed for the program and the hours submitted on the renewal application for 2015 indicate that the program operates from 7:30 a.m.-5:00 p.m. Monday through Friday with no documentation or notification that the hours of operation had changed. A second visit was attempted on 1/26/16. An interview with the director confirmed that when school is in session the program operates from 3:40 p.m. to 5:00 p.m. and on days when there is not school including summers the program operates from 7:30 a.m.-5:00 p.m.

An annual check of the: a. Kentucky driver history records in accordance with KRS 186.018; or b. Driver history records through the state transportation agency that issued the driver’s license; (c) Hold a current driver’s license which has not been suspended or revoked during the last five (5) years; and (d) Not caused an accident which resulted in the death of a person.
