General: Based on interview, on 09/09/24 a staff person was vacuuming after lunch and a two-year-old child kept wrapping the vacuum cord around his neck; the child was strapped in a standard size highchair without the tray and given a toy to occupy him until vacuuming was completed. A second staff person was changing a diaper. Staff stated that the child was in the high chair for approximately five to ten (5-10) minutes. While in the high chair, another child came over and pushed the highchair over, and the child landed on his face. The child went to the hospital and was diagnosed with a fracture to his nose. General: Based on observation, the surveyor found the following: Toddler 1 Classroom: 1. When the surveyor arrived to the child-care center, the surveyor conducted a head count in the Toddler 1 Classroom at 11:14 am and found that there were nine (9) children with the youngest child being two (2) years old with two (2) staff persons present. At 11:22 am, the surveyor observed two (2) staff exit the Toddler 1 Classroom and enter a side room that was connected to the Toddler 1 area. The surveyor observed both staff to re-enter the classroom approximately twenty (20) seconds later. Toddler 2 Classroom: 1. The surveyor conducted a head count in the Toddler 2 Classroom at 11:14:30 am and found that there were ten (10) children with the youngest child being fourteen (14) months old with two (2) staff present. At 11:19 am, the surveyor observed one (1) staff person to leave the classroom and enter a side room that was connected to the Toddler 2 area. The surveyor observed the other staff person to be alone in the classroom from 11:19 am until 11:20:59 am. When the surveyor reviewed employee documentation, the surveyor found that the staff person who was left alone in this classroom was under the age of eighteen (18) years of age. 2. When the surveyor re-entered the classroom at 12:33 pm, the surveyor observed a staff person leave the classroom and enter a side room that was connected to the Toddler 2 area approximately twelve (12) times in a three (3) minute span. During the time that she was leaving the classroom, the other staff person, who was under age eighteen (18), was again left alone with the children. Therefore, the child-care center failed to ensure adequate supervision by a qualified staff throughout the child-care center at all times. There was a long grey cord plugged into a wall; the cord was connected to a telephone located on a white shelf. The surveyor observed the cord to not be secured and could easily be pulled. Infant Classroom: 1. There was a baby swing placed next to the door ; the swing had a long cord plugged into the wall that was not secured. School Age Classroom: 1. There was a cardboard box in the back right corner that contained a yellow screwdriver and a pink power drill. 2. There were pieces of wood that appeared to be pieces of a shelf that were placed against the wall. During interview with staff-in-charge, she stated that the pieces of wood belonged to a food play set that she was putting together for the Preschool Classroom. 3. There was a large printer that had cords plugged into the wall that were not secured. 4. There was a Febreeze Wall Plug In that was accessible to children. The surveyor looked up the packaging for the plug in and the packaging stated, "Keep Out of Reach of Children." School Age Rest Room: 1. General: Based on observation, the surveyor found that the indoor gross motor area was being used as a School Age Classroom on the day of the surveyor's visit. The surveyor found that there were seven (7) children ages five to nine (5-9) years of age utilizing the space as a classroom during the summer months. During interview with staff-in-charge, she stated that when children from other classrooms needed to utilize the indoor gross motor area, they would keep the children separated during the scheduled times. Therefore, the child-care center failed to notify DRCC of changes to the diagram that would increase a change in capacity for the child-care center.

The surveyor conducted a walk-through of the child-care center and did classroom observations. The surveyor walked back into the School Age Classroom and observed three (3) children to place their tablets down on a table at 12:10 pm and play a group game. The surveyor observed one (1) child to stay on her tablet for approximately one (1) hour and twenty (20) minutes; therefore, the child-care center failed to ensure that children were not on electronic devices for longer than one (1) hour.

General: Based on interviews, the surveyor learned that the child-care center failed to report an incident to the Division of Regulated Child Care (DRCC) within twenty-four (24) hours when a child received an injury that required medical attention. On 10/25/23, a one-year-old child’s nose began bleeding when the child was bitten on the nose by another child in the Toddler Classroom. Staff at the child-care center notified the child’s parent of the incident and the child was taken for medical treatment. Staff members confirmed during interviews that they were aware that the child received medical treatment. Staff member thought the incident did not have to be reported to DRCC since the child-care center did not send the child for medical treatment. General: Based on observation, the surveyor observed a staff person from the One Year Old Classroom to enter a restroom located by the One Year Old Classroom and Two Year Old Classroom at approximately 12:56 p.m. The surveyor observed there to be no staff person inside the One Year Old Classroom. The surveyor observed the staff person from the Two Year Old Classroom to walk into the One Year Old Classroom, leaving the Two Year Old Classroom without a staff person. The surveyor observed another staff person to walk back to the One Year Old Classroom and Two Year Old Classroom at approximately 12:58 p.m. and go into the One Year Old Classroom, allowing the other staff person to go back to the Two Year Old Classroom. Therefore, the Two Year Old Classroom was observed to have a qualified staff person at approximately 12:58 p.m. The One Year Old Classroom and Two Year Old Classroom was observed to be a large room that was divided down the middle by small child sized shelving. The One Year Old Classroom was observed to be on one (1) side of the shelving and the Two Year Old Classroom was observed to be on the other side of the shelving. During interview, staff-in-charge stated that the staff person in the One Year Old Classroom got sick and had to go to the restroom to vomit.
