Stacks of totes that contained what appeared to be toy items placed on a blue bench located in the hallway by the exit door that leads to the playground. The totes were observed to not be secure and could easily be pulled off the bench. 2. Large plastic zip lock bags placed inside a bottom cabinet in the One-Year-Old Classroom. The safety lock on the cabinet was observed to be broken. 3. The surveyor found two (2) dead flies on the floor, one (1) on a mat, and one (1) in a small doll house in the Three Year Old Classroom. 4. A red cable was observed coming out from behind a cabinet and ran under a rug in the Three Year Old Classroom. The cable was observed to not be secured. 5. An unsecured black cord was observed to be hanging down from behind a wall mounted television in the Three Year Old Classroom. 6. Three (3) clear totes that contained what appeared to be clothing items were stacked and placed in the bathtub located in the restroom next to the Preschool Classroom.
