General: Based on interview and review of documentation, the surveyor found that a staff member gave milk to a child that has an allergy to milk and should receive almond milk. The child had an allergic reaction, i.e., her face turned red, her face swelled, and her throat swelled. The child’s allergy information was exposed and posted on the wall. Staff used an EpiPen for the child, called mom, and staff called an ambulance. During interview, staff stated they knew the child had an allergy to milk and that allergy information was available on the classroom wall. During a tour of the childcare center, the surveyor observed the allergy information posted in the classroom. Therefore, the child’s health, safety and comfort were compromised.

Interview with a staff member (DOH: 08/20/01) found that the contract employees take the children to an office area that is not part of the child care center's licensed space to provide services. Nursery 1: Based on review of documentation and interview, the surveyor found that a three-year-old child arrived at the child care center at 8:10 a.m. and was transitioned into another classroom at 9:30 a.m. Through staff interview, the surveyor learned that the child did not receive a mid-morning snack and ate lunch sometime between 11:20 a.m. and 11:30 a.m.; therefore, the child was not fed within three (3) hours of his last meal.
