Nursery 1: Based on review of documentation and interview, the surveyor found that a three-year-old child arrived at the child care center at 8:10 a.m. and was transitioned into another classroom at 9:30 a.m. Through staff interview, the surveyor learned that the child did not receive a mid-morning snack and ate lunch sometime between 11:20 a.m. and 11:30 a.m.; therefore, the child was not fed within three (3) hours of his last meal.
