General: Based on review of documentation found in the employee personnel files and review of ECE-TRIS, the surveyor discovered the following: 1. One (1) staff (DOH: 4/1/1995) completed the cabinet-approved pediatric abusive head trauma training on 4/25/2017. 2. One (1) staff (DOH: 1/2/2002) completed the cabinet-approved pediatric abusive head trauma training on 4/21/2017. 3. One (1) staff (DOH: 9/29/1996) completed the cabinet-approved pediatric abusive head trauma training on 4/24/2017. Therefore, the child-care center failed to ensure that the employees completed one and one-half (1 ½) hours of cabinet-approved pediatric abusive head trauma training once every five (5) years. General: Based on review of documentation, the child care center has failed to comply with the regulatory requirements for a driver transporting children for the center. The files presented for two (2) child care staff persons (dates of hire: 4/2/1992 and 10/22/2007) did not contain an annual check of the Kentucky driver history record in accordance with KRS 186.018 or the driver history record through the state transporation agency that issued the driver's license. General: Based on observation, the emergency hatch on the bus was leaking. There was water coming in and making the seats on the bus wet. The center arranged for another bus to take the children home that afternoon. However, the children rode that bus in the morning and it had been raining all day.
