The director approached the surveyor upon sight. When the surveyor entered the Infant Classroom at 3:02 pm, one (1) staff member was caring for four (4) infants. Based on interview, the surveyor found: The director acknowledged that the surveyor informed her of being named as an alleged perpetrator and of the regulatory requirement to be removed from direct contact with a child in care on 01/26/24 at 2:52 pm. She reported she was speaking to a staff member in the Infant Classroom when the surveyor arrived and was not involved with children in care. Based on review of video camera footage, the surveyor found: The director entered the Infant Classroom on 01/31/24 at 2:51 pm, spoke to a staff member, and exited the Infant Classroom at 2:59 pm; the director did not remove herself from direct contact with children in care. General: Based on review of video camera footage of the 2-Year-Old Classroom and based on interview, the surveyor found the following: A two-year-old child bit a three-year-old child eleven (11) times within a two-day period (01/11/24 and 01/12/24) while in the 2-Year-old room. Two (2) staff were present in the classroom; however, they failed to adequately supervise the children in the classroom. Additionally, based on video footage for 01/11/24 with a time stamp duration of 3:54 pm-3:59 pm, two (2) staff members were out of view of the video camera due to blind spots near the entry door and diaper changing area wall. A three-year-old child climbed on a chair and took a staff cup from the top of a shelf. A two-year-old child approached the three-year- old child and attempted to take the cup from the child. The three-year old child climbed down from the chair with the cup. Both children struggled over the cup and the two-year-old child bit the three-year-old child on the chest. The three-year-old child walked with the cup across the classroom to a couch and sat down, followed by the two-year old child. The three-year-old child drank from the cup. The two-year-old child took the cup and drank from the cup. The two-year-old child drank from the cup 3 (three) more times before a staff member walked over to take the cup. The children were not provided with adequate supervision. The director was present during the video footage review, and confirmed the cup belonged to staff and contained water. She reported both staff were present in the 2-Year-Old room attending other children or at the entry door on 01/11/24 from 3:54 pm-3:59 pm.
