General: Based on observation and interview, of the two-year-old room, there was not an adequate amount of developmental materials and toys for the children to play with. There were five (5) fabric toy cars, one (1) plastic green ball, and two (2) fabric story books. There were six children present in the room at the time of the observation. Further observation, revealed in the same room was an area the was sectioned off with a gate and divider that led to a space with large plastic blocks, age appropriate story books, cars, dolls, balls and manipulatives that were age appropriate for the children. Interview with the staff member revealed that she gets the toys out through out the day and puts them back in the gated area where the childen can not gain access of the matrials. Interview with the staff in charge is aware of this concern and has coached the staff member in regards to this concern.

Per interviews, the center failed to assure the health, safety, and comfort of a three-year-old child while in the classroom. A three-year-old child was playing in the kitchen area when another three-year-old child approached them and stuck a bead into the child’s ear causing it to be dislodged in the ear canal. The three-year-old child was caused discomfort, and had to have the bead removed by a physician from the emergency room. Further interview with the director and staff member revealed the bead was retrieved from a previous art project that had been glued to a paper crown.

Based on observation in the infant room, a ten month old male infant was continually crying without any attention by the staff member. After observing the infant crying for approximately ten minutes, without recognition from a staff member, the surveyor asked why the infant was not being attended to, the staff member stated, "he cries all day every day". After approximately fifteen minutes, the surveyors left the room and the infant was continuing to cry. Approximately 30-45 minutes later, the surveyors returned to the infant room to find the infant still crying. At this time, a staff member was trying to feed the infant, even though he was still crying and very visibly still upset. The staff member tried more than a few times to insert food into his mouth, but he would turn away from the spoon. The staff member also stated during an interview that she did not know if the infant took a bottle. Approximately 30 minutes later, the infant could no longer be heard crying and had fallen asleep.

Based on a review of the documentation, there was unavailable for review, a lesson plan/activity plan which detailed activities which are individualized and developmentally appropriate for each child served that provides experience to promote the individual child's physical, emotional, social and intellectual growth and well-being including a variety of activies including: art, music, dramatic play, stories and books, science, block building, tactile activity, culture, indoor and outdoor play and a balance of active and quiet play. Based on an interview with a staff member, the authors that were listed as the lesson plan for the day had been used earlier in the week and today's lesson plan she had just "winged it". General: Based on Observation, the child care center failed to maintain equipment in accordance with regulatory requirements. Based on observation, the outdoor play equipment including a pink, white and blue riding toy had a plastic back which was broken along with broken handle grips, a Little Tykes walker had a broken knob which exposed a sharp, piece of plastic extending out the front, along with a two inch screw and was in disrepair along with several small toys including a push mower, and a Little Tykes trailer. Based on continued observation, a red seated metal tricycle had no pedals. The handrail on the climbing structure was loose and was not sturdy and secured.

Continued observation revealed that at the end of the bench, where the 2 X 4 would be attached were 2 rusty nails protruding upward, creating an extremely hazardous seating surface.
