General: Based on Interview, the facility failed to provide for the health, safety and comfort of each child. Allegations involving a five (5) year old child receiving an injury to his head and scraps/scratches on his legs that had not been cared for while in the care of the facility were investigated on 8/5/14. During interview staff confirmed the allegations and stated that the incidents occurred in the facility parking lot. A staff person that was present when the head injury incident occurred described what happened by stating, "We mark off a circle in the parking lot to show the children where they can ride their bikes. We told him to stay on the sidewalk so he would be out of the way of the children riding their bikes, but he got up and moved into the bike path to draw with chalk without us knowing. As soon as he sat down in the bike path I saw a child run into him with their bike. I think the bar on the base of the child's bike hit the side of his head and he started crying immediately. I escorted him out of the bike lane and asked a staff person to get ice. He had a red mark on the side of his head, but no other visible injuries". The staff person that was present when the injury occurred involving scrapes/scratches to the child's legs stated, "I was outside with a group of school age children and they were running around playing tag on the blacktop. He was running really fast when he tripped, fell and scrapped his knee. I sat him down on the sidewalk to talk to him and calm him down. Looking back on that now, I should have probably got him a Band-aid; I need to start carrying them with me". The staff person described the injury by stating, "He had a large scrape on one of his knees. Blood came to the surface, but it wasn't running down his leg". The staff person further stated that she was working alone with the group and stated that she did not write an incident report because she forgot. The staff person stated that she leaves around 3:00PM each day; therefore, she was not present when the child's mother arrived to pick him up. Even though staff stated that the child appeared to be fine after the bike incident, the parent was not made aware of the head injury until she arrived at the facility to pick up the child that afternoon. The parent took the child to a physician to be examined as a result of the incident. Furthermore, proper first aid treatment was not provided to the child when the child fell and scraped his knee in the facility parking lot.

Vehicles use Hidden Valley Road to access an apartment complex and a BP gas station. Vehicles use the smaller street to access the post office parking lot. One staff person stated during interview that staff cars are used to block off the area in the parking lot where the children play; however, additional staff interviewed only identified chalk lines being drawn on the blacktop and indicated physical barriers are not used.
