One staff person hired on 1/3/20, was observed working with another staff person in the infant classroom during the visit. This staff person had a completed criminal records check (CRC) and a child abuse/neglect check (CAN) on file; however, the facility has yet to initiate the process for obtaining a background check for this staff person through the Kentucky National Background check service (KARES). 2. Some of the children commented the surveyor must be using the restroom and laughed. This same staff person was overheard telling a child to sit in a chair for timeout. The child began to cry. The staff person repeatedly told the child they were being a baby because they were crying. General: Based on observation, during a tour of the facility the following was found: 1) In the Three's Room a large area of paint had peeled away from the surface in the corner where an area rug with a road imprinted on the surface was located. Part of the drywall was also exposed. There was a space under the windows where the paint had peeled away leaving the drywall exposed. Scuff–marks and peeling paint were found on the wall located where a wire rack is hanging. A wall with the words Happy Birthday on it had an area near the baseboard that had areas of chipped paint and sticky tape residue with an accumulation of dirt and grime. 2) In the Four's Room white paint had been used to paint the front and back of the half door leading into the bathroom. The paint had peeled away from the surface on both sides of the door leaving the brown wood surface exposed. A portion of the carpeted baseboard was missing at the bottom of the wall by the bathroom. In the Science area, numerous scribble marks were found on the surface of the window frame. Areas of peeling paint were found on the wall located in the area used for Circle Time. The gray paint on the door facing leading to the playground had peeled off the surface in several areas leaving the black paint beneath exposed. 3) In the Two's Room there were numerous areas on the walls and under the window where the paint had peeled away from the surface. 4) In the One's Room, areas of chipped paint were observed on the wall by a side exit door. Dirt, grime and dust were observed along the tops of the baseboards and windowsills.
