General: Based on observation, during the walk-through of the Two-Year-Old Room the surveyor found the following: 1. Dark marks and various types of debris were on the floor and on a rug in the classroom; therefore, the floors in this room were not kept clean. During interview, staff-in-charge stated that the debris on the floor was from their Christmas tree decorations, and that the room was not currently in use due to low numbers of two-year-old children. Staff-in-charge stated that she would have the floors cleaned. 2. Several areas of chipped and/or peeling paint were on the face of the heat register located within reach of children; therefore, this was not kept in good repair. General: Based on observation and interview, this regulatory requirement was not met. The following was found: 1. Upon arrival (8:05AM), the surveyor approached the facility’s entrance door while a parent was waiting at the door to drop off her child. A staff person came to the front door to open it and retrieve the child from the parent. The surveyor followed the staff person down the hallway and into the Two Year Old Classroom. A tour of the classroom revealed that five children (two - five years old) had been left without adult supervision while the staff person left the room to open the door for the surveyor. All of the children were awake and playing freely in the classroom. It is important to note that the distance between the facility's entrance door and the two year old classroom doorway is approximately 13 - 15 feet; therefore, the children in the classroom were not visible from the facility's entrance door. 2. As the surveyor followed the staff person down the hallway upon arrival, two staff turned the corner at the opposite end of the hallway and started walking toward the surveyor. One staff person entered the infant classroom and the other staff person entered the two year old classroom. The surveyor entered the infant classroom and found that three children (two infants and one school age child) had been left without adult supervision while the staff person left the room. The staff person stated that she had left the room to open the door for the other staff person who had just arrived. It is important to note that the staff person had to walk approximately 11 feet down the main hallway, turn a corner and walk approximately 9 - 10 feet, and then turn a corner again and walk approximately 7 - 8 feet to access the door she opened. Staff stated that parents fill out a permission form to administer the epi-pen and other medications. It was also found that there had been a child that had a bottle of Benadryl at the facility in case of an allergic reaction. At the time of the investigation three (3) epi-pens were observed stored in a closet in the three to five (3-5) year old room. A review of documentation found that two (2) of the children did not have a written request to administer the medication on file at the facility. The third child had a form that had been filled out but was not signed by the parent or guardian. The director stated in interview during the exit conference that the child who had the Benadryl had a permission form but they no longer have the documentation. 2) In the infant room two (2) tubes of diaper ointment were observed in open containers on the middle shelf of the changing table. The surveyor asked staff if there was written permission on file for the administration of the diaper ointment. Interviews with staff found that children in the three to five (3-5) year old room take a walk approximately two (2) times a week. The staff person stated she has taken the children for a walk around the block and down the street to see a waterfall. Further interviews with staff found that on 6/6/19 the Snow Cone bus came to the facility. The children in the two-to-three (2-3) year old room and in the three to five (3-5) year old room went out into the parking lot to the Snow Cone bus. In addition it was found that children in the two-to three (2-3) year old walked to a family member’s home for an Easter egg hunt. A review of documents found there was no written permission from parents for the children to walk to the family member’s home for the Easter egg hunt. There was not written permission from parents for the children to be in the parking lot which is not fenced when the Snow Cone bus was at the facility. There was no written permission from parents allowing there child to walk around the block or to see the waterfall down the street. An interview with the director found she has parents fill out a blanket permission slip. The surveyor reviewed the document parents were asked to sign. The document did not specify a date the children would be outside of the facility and it did not specify the time they would leave the facility or return. Interviews with staff also stated there are no specific days of the week and times they take the walks.

A review of Division of Regulated Child Care files found that this regulatory violation was previously issued as a result of an investigation conducted on 2/25/19. The owner/director had indicated on a Plan of Correction submitted 3/11/19 that they had removed the structure from the playroom and listed 2/26/19 as the completion date.

General: Based on observation and interview, this regulatory requirement was not met. Upon arrival (11:15AM) the owner/director escorted the surveyor on a tour of the facility. During the start of the tour the owner/director identified a group of two (2) year olds that were observed sitting in the hallway with a staff person that was observed changing diapers at the diaper changing table. After proceeding down the hallway and conducting a tour of three (3) classrooms the surveyor returned and observed two (2) children (two (2) year olds) playing under a classroom table in the two (2) year old classroom. The owner/director immediatley entered the classroom and instructed the two (2) children to rejoin their group in the hallway. The surveyor entered the two (2) year old classroom and found that no adults were present in the classroom; therefore, the children were unsupervised. The staff person in the hall had nine (9) children (two (2) year olds) present in her group at the time. The diaper changing table is positioned approximatley fifteen (15) feet down the hallway from the doorway of the two (2) year old classroom. The two (2) children playing under the table were not visible from where the staff person was positioned.
