General: Based on interview and review of documentation, including video footage, the surveyor found the following: 1. On 01/14/26, a sixteen-month-old child sustained first- and second-degree burns when another child pulled an accessible, unsecured cord attached to a Dr. Brown's Bottle Warmer, the warmer came off the counter, and hot water splashed onto the child. Unknown to the staff member present, the two (2) children were playing with the unsecured cord at the time of the incident. The video footage showed that the child immediately began to cry; the staff person turned around, picked up the bottle warmer, and then picked up the child to clean her face. The child continued to cry, obviously in pain. Later, the child was taken to the emergency room and diagnosed with first- and second-degree burns; therefore, the health, safety and comfort of the child was compromised. 2. On 01/14/26, the Nursery staff person denied having knowledge of hot water from a bottle warmer being splashed on a child. The staff person originally told the staff-in-charge and parents that the child was constipated and later informed parents that the child had suffered a reaction due to diaper rash cream. However, details of the incident were later discovered after review of video footage. Due to the staff person not informing the parents or the staff-in-charge of the incident, the child was not promptly given proper medical attention; therefore, the health, safety and comfort of the child was compromised.

General: Based on Observation, and interview, the portable sink used by the staff and children in the two (2) year old classroom in Building #2 was not working properly. During a tour of this classroom the surveyor found that the sink was not operable. The staff person in charge and the staff person working in the classroom stated that the sink had not been used for a couple of days; therefore, was unplugged. The staff further explained that a toy was stuck inside the drainage tube which was causing the sink to drain slowly.

General: Based on Observation, the portable sink used by the staff and children in the two (2) year old classroom in Building #2 was not equipped with hot water. The surveyor tested the temperature of the water with a calibrated thermometer and found it to be eighty-six (86) degrees Fahrenheit which does not meet the minimum requirement.

The surveyor observed 2% milk being served to ages twenty-five (25) months and older during lunch.
