General: Based on observation and interview, the following was found: 1) The surveyor entered the facility at 9:19 am and observed one (1) staff member caring for a group of eight (8) children in the Indoor Gross Motor Room 2 (front of building). The children were identified as all being one-year-olds. The minimum staff to child ratio would need to be one staff member for six (6) children; therefore, the facility exceeded the staff/child ratio by two (2) children. It is important to note that the surveyor observed three (3) different staff rotate in and out with this group of children throughout the entire inspection; however, only one (1) staff member cared for the group at a time. 2) During the initial tour of the facility with the staff member in charge, the surveyor observed one (1) staff member caring for a group of fifteen (15) children in the Preschool Classroom and then transition this group of children to the Indoor Gross Motor Room 1 (back of building). The children were identified as being three to five (3-5) years old. Since the youngest child in the group was a three-year-old, the minimum staff to child ratio would need to be one staff member for twelve (12) children. The staff member in charge identified a second staff member that was assigned to this group; however, the second staff member was on her break at the time. The staff member was observed holding a child, identified as being her own, in the front of the building upon the surveyor's arrival.

The highchair had a broken leg. 3) An adult chair with a padded seat on the playground was in disrepair. The fabric on the seat area was torn which exposed the foam filling. 4) Two (2) seats on a metal trike merry-go-round on the playground had cracked/broken plastic which created sharp edges and pinching hazards.

The staff person in charge stated that she has a portable sink on wheels that she moves into these classrooms during diaper changing times. The portable sink used by the staff and children was not equipped with hot water. The staff person in charge confirmed during interview that the sink does not contain a heating element to change the temperature of the water to the minimum requirement. 2) There were no paper towels or hand drying materials at the sink in the Infant Classroom. The staff member working in the classroom stated she was getting ready to text someone up front and request for them to bring some paper towels to her.
