General: Based on observation, while the surveyor was conducting a head count in the Infant Classroom at 12:11 pm, the surveyor observed a four-year-old child to open the office door that was connected to the Infant Classroom and walk into the Infant Classroom. The surveyor observed the child walk through the Infant Classroom while stating he was returning to his classroom. The surveyor did not observe a staff person in the office with the child and observed the child walk alone from the Infant Classroom to the Preschool Classroom. During interview with the Infant Classroom staff person, she stated that the child was the director's child and had been sitting in the office for a few minutes. During interview with the director, the surveyor found that the child had been left alone in the office for approximately three (3) to four (4) minutes alone without a staff member present; therefore, the child-care center failed to ensure adequate supervision throughout the child-care center.
