Two (2) staff members (DOH: 09/09/24 and 10/21/24) did not complete the required Pediatric Abusive Head Trauma training. 3. Three (3) staff members (DOH: 03/19/25, 06/16/25, and 08/19/25) did not complete orientation training within their first three months of employment. A review of ECE-TRIS confirmed these findings.

General: Based on observation and interview, a one (1) year-old child was sleeping on the floor in One's Room. The staff person stated that she entered into Brightwheel when the child fell asleep. After looking in the app, she stated that child fell asleep at 9:54 am. The surveyor entered the One's Room at 10:15 am; therefore, the child was asleep on the floor for twenty (20) minutes and was not provided a mat or cot.

General: Based on observation, interviews, and a review of documentation, it was determined that children in the One-Year-Old Classroom were subjected to electronic viewing throughout the day. Children watched the program “Bible Buddies” daily from 8:45 a.m. to 9:00 a.m. Staff members also reported using electronic viewing during scheduled Music and Movement from 10:30 a.m. to 11:00 a.m. During a surveyor visit at 10:57 a.m., the television was observed playing “Ms. Rachel.” The staff member present stated that the video was played during the lead staff member’s ten (10) minute break. It should be noted that electronic viewing is prohibited for children under twenty-four (24) months of age.

Based on interview and observation, the following was found: 1.) A four-year-old (4) child eloped from the playground, ran through the building and exited through the front door on 04/22/25 at approximately 5:00 pm. The staff members inside the childcare center were not aware that the child had exited the building and continued to search for him inside. A parent leaving the childcare center observed the child searching for a toy that had been thrown over the playground fence. The parent was able to coerce the child back into the building. It was reported that the staff did not know the whereabouts of the child for approximately three (3) minutes. 2.) During a tour of the childcare center on 04/24/25, the surveyor entered the One-Year-Old Feeding Room and observed four (4) children seated at a sit-in feeding table with a staff member. Two (2) one-year-old children were found sleeping in the One-Year-Old Classroom, outside of the Feeding Room. It is unknown how long the sleeping children were unsupervised.
