Observation revealed two ceiling tiles that were stained and failed to be in good condition located in the Three Year room. Additional observation revealed three stained ceiling tiles located in the Four Year room that failed to be in good condition at the time of the survey. Interview with the director revealed that the new ceiling design had been delayed, but that all of the tiles were being replaced with acoustic clouds.
