Based on observation and review of documentation, the surveyor viewed the video footage of an incident that occurred between a staff member and a four-year-old child. The surveyor confirmed that the child was four-years-old by reviewing the child’s enrollment packet. In the video, the surveyor observed a staff member say something in the video and walk over to the child as the child picked up a blue square container that contained blocks. The child dropped the blue square container with blocks and the container fell to the ground. The staff member took her right hand and grabbed the child’s left arm between the child’s wrist and elbow. The staff member started walking while pulling the child by her left arm. The staff member lifted the child up by her arm over top the blocks and the child’s feet lifted off the floor. The child’s feet landed on the floor on a blue circle rug as the staff member continued to pull the child by the arm. The child appeared to go limp as her knee’s hit the hard surface flooring. The staff member continues to pull the child by her arm across the floor. At times, the staff member lifted the child off the floor while holding the child by the arm prior to the staff member placing the child on the floor on her bottom with the child’s back against the wall. After viewing the video footage, the surveyor went into the classroom where the incident occurred between the staff member and the child. The surveyor measured that the staff member pulled the child by her arm and lifted off the ground multiple times by her arm for approximately twenty-five (25) feet. During interview, staff-in-charge confirmed the incident occurred as the staff-in-charge viewed the incident on video footage and the staff member is no longer employed by the child care center. Based on review of documentation, DCBS provided DRCC with a letter that confirmed a substantiated finding on the staff member.
