General: Based on interview, this regulation was not met. A child in the Infant Classroom found and used a vape pen/device, that was brought into the center by a staff member. She dropped the vape on her way to take a break when it fell out of her purse. A child found the vape pen/device and took a "hit" from it. An ambulance was immediately called, and the child was evaluated for any health concerns. The child did not suffer any injuries, and the parents were also notified of the incident.

General: Based on interview, record review and review of video surveillance of the playground on 07/24/2020, a one-year-old child received 2nd degree burns to the bottom of her feet. The child went outside at approximately 4:03pm with three other children and the staff person. The child did not have socks or shoes on her feet. The other children had on shoes while on the playground. The video shows the child walking around and playing, then at 4:18pm she comes into camera view walking onto an area of concrete that was directly in the sun. She walks on the concrete in this area for approximately forty (40) seconds and then walks over to a poured rubber mulch section of playground, next to a play structure, and falls down. She is seen getting up, trying to walk and falling down numerous times. At 4:19pm the child's teacher sees her, picks her up and she gathers the other children and goes back inside to their classroom. Video surveillance of the classroom at 4:21pm shows the children coming back inside from the playground and it appears the child without shoes on is still in distress. Her face is very red, she appears to be crying and having a difficult time walking so the teacher picks up the child. Over the next twenty-five (25) minutes, the teacher is observed, via video, trying to console the child by holding her, feeding her, giving her a pacifier and swaddling her but the child continued to appear in distress. At one point, another staff person comes into the room to give the teacher a ten (10) minute break but does not see the burns to the child's feet. At another point in the video, the child is observed lying on the floor crying, face down to the floor. At 4:45pm, the teacher appears to be putting socks and shoes on the child. The child eventually manages to get her shoes off and at approximately 5:30pm the teacher goes out of view with the child for a moment and when she reappears the child does not have socks on. The teacher is then seen showing the bottoms of the child's feet to the teacher who is in the adjacent classroom. It was reported the teacher tried sending pictures to the mother through the classroom IPad but it was not working properly. At 5:35 pm, the video shows the teacher applying some kind of ointment to the child's feet. Per interview with the teacher, she stated that she did put diaper ointment on the child's feet but immediately took it off. At 5:38pm the teacher called to the front office and asked for someone to come to the classroom. At 5:45pm the child was taken up front by another staff person and parents were called. The parents arrived to pick up the child at approximately 6:00pm. According to AccuWeather, the outside temperature that day was ninety-one (91) degrees. In the infant B classroom, there were two large areas of missing paint. These areas were located behind where the diaper changing/handwashing steps were hanging, as well as underneath the dry erase board in the classroom. 3. In the infant C classroom, there was an area of the wall where the drywall was completely missing, which left the metal in the wall exposed. 4. In the toddler B classroom, there was a brownish-yellow substance on the wall in the back left hand corner. 5. In the transition A classroom, there was an area of the wall where something had been hanging previously. The item was no longer hanging on the wall, which resulted in there being a large area of missing or mismatched paint. General: Based on observation and review of documentation, a five (5) year old child was taken into an isolation room after showing aggressive behavior towards another child. There was a video camera in this isolation room. A review of video footage found the staff person was sitting in the isolation room with the child when he began to climb on a table and appeared to try and bite and get aggressive towards the staff person. The staff person opened the door and stepped partially out into the hallway to ask another staff person to contact the child's father. At that time the child grabbed the staff persons arm and appeared to try and bite the staff person. The staff person reacted and pushed the child off of her causing the child to fall backwards and land on the floor against a chair. The child is then seen grabbing his head. Through interview with staff, it was found that the child received a bump on his head. It should be noted that the child has a shunt in his head due to a medical condition. The child was taken to the hospital a few days after the incident due to an illness not related to this incident. During that visit the doctor was informed about the incident where the child hit his head so an x-ray was completed. It was reported that there were no injuries from the fall. General: Based on Review of Documentation, it was found that on 1-31-17, a child tripped over a rug and hit his head on the metal casing of a window, resulting in a gash on the left side of his forehead. He was taken to the hospital and required three (3) stitches to close the wound. On 2-15-17, a child fell and bit his tongue. He was taken to the hospital and required two (2) stitches to close the wound. The facility personnel were aware of the children being taken to the hospital for treatment. Neither of these incidents were reported to the Division of Regulated Child Care office as required.
