At approximately 3:00 p.m., one (1) of the staff left the cafeteria and went to the school office leaving the other staff person with the twenty-six (26) children. The staff to child ratio with the age of the youngest child being five-years-old is one (1) staff for fifteen (15) children. The date of birth of the youngest child present at that time was 10/23/13. Based on observation the surveyor observed a total of thirty-two (32) children ages five to eleven (5-11) years old present in the cafeteria. The surveyor noted the facility has a licensed capacity of thirty (30). The staff person in charge stated two (2) children who are siblings were not supposed to attend because they are only to attend two (2) days a week. She stated they were present on Monday and Tuesday of this week and should not have attended today.
