General: Based on observation and interview, it was found that parents provide their child's lunch each day. Lunch was observed in both classrooms at the time of the survey. The surveyor found that some of the children's lunches did not have all of the required food components for lunch. For example, one child had peanut butter and crackers and an Uncrustable sandwich that contained peanut butter and jelly. The child was missing milk, two servings of a fruit or vegetable or one serving of a fruit and one serving of a vegetable. Another child's lunch contained Cheetos, crackers and cheese, a Nutri-grain Bar and a small portion of milk. This child's lunch did not contain two servings of a fruit or vegetable or one serving of a fruit and one serving of a vegetable. An additional child's lunch contained Vanilla Wafers, Cheerios and a sandwich. The child was missing two servings of a fruit or two servings of a vegetable or one serving of each. A portion of a half-gallon of 2% milk was observed in the refrigerator in the back Preschool room. A staff person in the back Preschool Room was observed checking children's lunches and writing down missing components. The staff person stated that additional items used to supplement the lunches are stored in the kitchen down the hall. The staff person stated she would have to wait until another staff person came in the room so she could get the needed items. However, the items were not obtained and served to the children. The staff person in charge stated the staff person had a criminal records check processed by the Catholic Diocese through a company named Virtus; however, this does not meet the regulation. Based on the COVID-19 emergency regulations 922 KAR 2:405E, new staff shall undergo name-based background checks (criminal records check and a central registry child abuse/ neglect check) upon hire and not be left alone with children until the name based background checks have been approved and returned.
