General: Based on Review of Documentation, it was found that the facility did not present evidence of submission for or completed criminal records checks and central registry checks for the facility's two (2) drivers. These drivers transport children by themselves on a daily basis. Due to dates of hire and prior addresses not being available, it could not be verified whether or not out of state background checks were required for these individuals. In addition, it was found that since January 2015, the facility utilizes two (2) volunteers for one and a half hours each week. These volunteers are students from the nearby school and are under the age of eighteen (18). These individuals interact with the children but are never left alone with the children. Neither individual has a central registry check or proof of submission for this check on file.
