General: Based on observation and interview, the following was found: 1) In the Preschool One (1) C Classroom, in a left bottom unsecured cabinet of a vanity, located on the back wall next to the refrigerator, there were staff purses that were accessible to children. 2) In the Preschool One (1) C Classroom, on top of the vanity located on the back wall next to the refrigerator, there were three (3) staff cups within reach of the children. 3) In the Toddler Two (2) Classroom bathroom, there was a staff member's purse located in a blue bin near the bathroom inside door. The surveyor verified with staff that at least one (1) child in the classroom uses the bathroom independently allowing her access to the purse.

General: Based on interview and review of documentation, during the investigation the following was found: On 07/10/25 at approximately 11:56 am, a staff member working in the Toddler 2 classroom did not appropriately redirect a two-year-old male child. Video footage revealed that the child was seated at a child-sized table when the staff member abruptly grabbed him by the left arm and moved him behind her while he remained seated in his chair. The child then scooted himself back toward the table while the staff member was bent over, wiping the table with a cloth. At that moment, the staff member extended her foot backward and kicked the child’s chair. This caused the child to fall out of the chair and onto the floor. He began crying but got back into his chair. Next, the staff member moved the child to the table and abruptly removed his glasses from his face. It should be noted that the staff member continued working in the Toddler 2 classroom for the remainder of the day, worked a full day on 07/11/25, and worked for two (2) hours on 07/14/25 before management acted. The staff member was terminated on 07/14/25. A report was submitted to the Cabinet; however, it was not made within the required twenty-four (24) hours of discovery. General: Based on Interview, a fire drill was conducted on 07/25/13 and the facility evacuated to the parking lot onto the pavement. At least two (2) children did not have shoes or socks on and stood on the hot pavement for approximately two (2) minutes. One (1) child received 2nd degree burns on the bottom of her feet.
