General: Based on observation, the surveyor found the following: 1) Based on observation at approximately 10:10 a.m., the surveyor found a female toddler sleeping in a blue cube chair. The child was somewhat slumped down in the chair, her head was down and turned to the left. The staff member told the surveyor that the toddler had been asleep for approximately five (5) minutes. The staff member made no attempt to move the toddler to a cot or a crib. The surveyor exited the classroom at approximately 10:20 a.m. Therefore, the surveyor noted the child had been in the chair for at least fifteen (15) minutes. 2) Based on observation at approximately 10:30 a.m., the surveyor found a safety risk in the Indoor Gross Motor Area and One-Year-Old classroom. The surveyor found several stacks of adult sized chairs positioned against the wall in the Indoor Gross Motor Area. During the observation, there were eighteen (18) children aged two-years-old playing within the general vicinity of the chairs. Additionally, the surveyor observed toddler sized chairs stacked on a toddler table in the One-Year-Old classroom. There were five (5) toddlers playing around the table.
