General: Based on observation, the surveyor found the following: One-Year-Old Classroom 1) A swing/sliding board structure was observed in the classroom without any fall zone material surrounding it. During the inspection, children were seen climbing up and down the slide, which was approximately two (2) feet in height. Three-Year-Old Classroom 1) Four child-sized chairs were stacked on top of a shelving unit located within the children's play area. Children were observed playing near the shelving unit, which posed a potential safety hazard. It should be noted that the staff member in charge removed the chairs during the exit interview.

Restroom 1) The tile next to the floor drain and toilet was cracked. 2) There was a build-up of dirt and grime embedded in the grout throughout the restroom. 3) There was a build-up of dirt and grime in the caulking around the toilet. General: Based on observation and interview, the surveyor found the following: Pre-K Classroom: 1) A brown water residue stain was observed on the ceiling in the corner of the classroom. 2) Excessive dust was found on the ceiling surrounding a circular exhaust vent. Three-Year-Old Classroom 1) Paper, food, and dirt particles were found on the carpet, with dirt embedded in several areas. The staff member in charge indicated that the carpet had been shampooed the previous week. Two-Year-Old Classroom 1) The baseboards were dented, covered with a significant build-up of dirt and grime, and had numerous areas of chipped paint. 2) There were several gashes and scuff marks on the walls. One-Year-Old Classroom 1) A large area of peeled paint was found directly behind a pretend play dress-up storage unit. Additionally, several gashes and scuff marks were observed on the walls.

General: Based on observation and interview, the surveyor found the following: One-Year-Old Classroom and Three-Year-Old Classroom: 1) The classrooms did not have an adequate amount of toys or programming materials available to the children. For example, while a dramatic play kitchen unit was present, there were no accompanying dishes, cookware, or pretend food items. Additionally, there were no multicultural materials or toys accessible in the classrooms. When the surveyor inquired about the lack of toys, staff were initially unsure. The staff member in charge later explained that the toys had been bagged up for approximately two (2) weeks due to a recent lice outbreak. The staff member showed the surveyor the bagged materials. In summary, there were no additional toys or materials available to support dramatic play or multicultural learning areas during the inspection.
